Please consider donating 1.2% of your paid income tax to support our cause

We invite you to become a sponsor of the Alytus District Municipality Primary Health Care Center by donating up to 1.2% of your paid income tax. Your support does not require additional expenses, all you need to do is fill out and submit the FR0512 form to the State Tax Inspectorate (VMI) by May 1, 2023.
When filling out the FR0512 application at: https://deklaravimas.vmi.lt/ please specify:
- The recipient's details of the income tax portion - 193107218 (company code);
- The percentage of the donation;
- The period in years;
- The purpose of the donation.
If you plan to donate for a longer period, please indicate the number of years (it cannot exceed 5 years).
