Information for residents about the procedure for issuing death certificates

When a loved one passes away, the person arranging the funeral must contact the deceased person's family doctor for the issuance of the medical death certificate.

For patients of the Alytus District Municipality Primary Health Care Center, medical death certificates are issued on weekdays from 7 AM to 6 PM, on Saturdays from 9 AM to 1 PM (contact by phone at +370 315 71412), and on Sundays and holidays from 10 AM to 1 PM (contact by phone at +370 687 51961).

Please note that a medical death certificate is issued in the following cases:

  • The patient dies outside a stationary healthcare facility when no suspicion of violence exists, and no forensic medical examination or pathological anatomy examination is required,
  • Based on available medical documents when the cause of death is clearly established and proven through medical research data,
  • Following the records of the person who confirmed the death, in the form No. 110/a "Emergency Medical Assistance Request Card".
